How To File Warranty Claim For Vet Bills
In the unfortunate event that you need to file a claim, please fill out the fields below and attach the following.
A copy of your Warranty Contract, Initial Exam, and an itemized veterinary bill.
Please ensure that the itemized bill from the treating veterinarian lists the identification for your pet, the procedures that were performed, and a diagnosis.
What is provided with the warranty program?
This program will reimburse you for veterinary bills that are resulting from illnesses attributed to the pet retailer. All illnesses must be diagnosed within 15 days of purchase. Any hereditary or congenital defects which affect the health of your pet (excluding dental) must be diagnosed within 1 year of your pet’s adoption.
Are common illnesses covered by the warranty?
Yes. The illnesses that are attributed to the pet retailer such as kennel cough, upper respiratory infection, pneumonia, parvo, and distemper, etc.
Concerning the health of my pet, which symptoms should I be aware of ?
Watch for coughing, runny nose, vomiting, loose stools. If any of these occur, contact your veterinarian. Make sure you provide food and water to your pet and monitor their consumption
Is the initial vet exam covered by the warranty?
The initial exam is the first visit to the veterinarian after adoption. It is not covered by the warranty.
We just completed the initial exam. What is next?
Keep all of your paperwork for the initial exam with your warranty contract. Make a copy of them to submit with your claim to the Administrator should you have a claim.
Will the warranty cover vaccinations, routeing procedures, or elective surgeries?
No. The warranty only reimburses veterinary expenses that are related to illness, and/or hereditary defects that affect the health of the pet. This excludes dental procedures.
What do I send to the claims Administrator?
Please submit a copy of the warranty contract, the veterinarian’s initial exam notes, and an itemized vet bill. Please ensure that the veterinary bill includes identification of the pet, the procedures that were provided, and a diagnosis.
What is the limitation on claims? When can I expect payment?
You have 60 days to submit a claim after the date of service. Once received, the Administrator will process your claim. This could vary depending on your claim. The processing is quicker if you have additional claims. You will not receive a confirmation when the Administrator receives it. However, if you do not receive anything or hear from the Administrator within 6 weeks, please contact the Administrator directly.
Can the warranty cover special foods, vitamins or specialized shampoos?
It does not.
Will the warranty cover trauma or accidents?
It does not.
Here is how you file a claim.
1.) Make sure you have a copy of all related documentation for your records.
A) Your purchase invoice or your warranty contract
B) Documentation from the exam with the diagnosis
C) An itemized bill from your Veterinarian
